Is this the right time to start your private dental practice? 7 Key Considerations in setting up Dental Clinic in Singapore.

There is a shortage of dentists in Singapore.

With a population approaching approximately 6.4 to 6.9 million within the next 5 years, there is a shortage of dentists in both the public healthcare institutions and in the private dental healthcare sector.

In a written answer for a query raised in parliament on 2nd April 2024 on the matter regarding ‘INCREASING THE NUMBER OF DENTISTS IN PUBLIC HEALTHCARE INSTITUTIONS TO MEET SINGAPOREANS’ DENTAL CARE NEEDS’, it was mentioned that “the number of dentists in the public healthcare sector grew by about 40%, from around 400 in 2013 to around 540 at the end of 2022. Going forward, we will continue to increase the number of dentists in public healthcare institutions to meet growing demand.”

1. Key Considerations in Setting Up Private Dental Practice in Singapore

Dr Kelvin Chye, Vice President of Singapore Dental Association (48th Council 2024-2026) mentioned in his presentation ‘National Oral Health Strategy A Sharing of Perspective from Singapore’ that as of 2022, dentists to population ratio was around 4.6:10,000.  According to the WHO registry, most developed nations would register ratios of above 6 dentists per 10,000 population.  Singapore still has room to expand dental services in the future – we have about 3050 dentists in practice, which means that to reach parity with developed nations, our dentists’ population would have to increase to around 3,600 to 3,800 – there is substantial growth potential for the dental service sector.

The other concern is the rising demand for dental care for the rapidly aging population in Singapore.

In the report ‘Population in Brief 2025’ released on Sept 29, 2025, the National Population and Talent Division (NPTD) mentioned that:

“the proportion of Singaporeans aged 65 years and older is rising, and at a faster pace compared with the last decade. About 24 per cent of citizens are projected to be aged 65 and above by 2030.”

Separately, in the State of Oral Healthcare in Singapore Results from the National Oral Health Survey 2019, it was reported that only ‘35.7% of those above 65 years of age are having 21 or more natural teeth – the desirable number of teeth for normal functioning.’  This implies that nearly two-thirds of those above 65 are in need of extensive dental care and intervention.  “Dental caries (cavities) experience was highly prevalent … with severe gingival inflammation and moderate periodontitis.”  

The report also mentioned that nearly four-fifth of those surveyed usually seek dental care in private dental clinics.

In view of shifts in demographics and demand patterns, the Singapore Government’s broad Oral Health Strategies for the future is to have ‘vertical integration between tertiary and primary care providers’ and to build up ‘robust primary dental care network through horizontal integration between public and private healthcare’ providers.

Dentists practising in Singapore who are currently considering launching their private dental care services will be able to obtain setting up and financing advisory through professional consultants, such as Medinex Specialist Centre, for their clinics.  While setting up a dental practice can often seem daunting, we have summarized the process under the following seven, though not exhaustive, key considerations:

7 Key Considerations in Dental Clinic Set Up ( The 7 S’s of Dental Clinic Set Up )

1. STURCTURE - Planning and Setting Up the Business Framework of Your Practice
  • Registering your dental business – in Singapore, to operate any business entity, whether a one-man proprietor or multiple shareholding company, you will need to, by law, register the business with ACRA – Singapore’s national regulator of business registration, financial reporting, public accountants, and corporate service providers (CSPs). This will involve choosing the appropriate name for your dental practice – while there are guidelines in using certain words such as ‘Singapore’, ‘National’ or ‘Temasek’, selecting one that represents your practice’s vision and values will be imperative in this phase.

  • Getting your practice license – beyond having a practice license from the Singapore Dental Council, getting the HCSA licence is a crucial step in starting your clinic operation. Promulgated on 6 January 2020 to regulate healthcare services in Singapore, the Healthcare Services Act (HCSA) is for providers who are intending to offer a Licensable Healthcare Service (LHS), which in this case is your dental clinic.  You will need to obtain this licence first before you are allowed to offer dental services in Singapore.
     
  • Like any business, determining your organization structure – sole proprietor, private limited, Partnership and LLC, etc. Shareholding structure ( who owns the practice? Dentist + investors) – is an important decision.  Drafting up shareholders agreement – document defining the relationship between shareholders, share transfer restrictions, and dispute resolution – is often a time of obtaining alignment in the values and objectives of the practice in the starting phase.

2. SERVICE - Managing Your Service's Operation & Financials

Knowing the business – how revenue is streaming in, flowing out and is being retained in the practice is paramount. Bookkeeping, accounting and fulfilling statutory requirements of filing financial documents is important not just for tracking the sustainability of the business but also for being accountable to the relevant authorities.

  • Know your Revenue – the profit margin of most general dental clinics ranges between 25% to 40%. Specialist dental services will tend to be higher due to more complicated surgical treatments.  Typically, in a dental clinic, a 60-30-10 distribution of dental service offerings would be observed.  Routine procedures such as scaling, polishing, extractions, dental consultation and fluoride services are usually the cash cows – i.e. the main revenue generator. Crown, fillings, root canal, paediatric dental, braces and surgical procedures will make up about 30%, with the residual 10% of the revenue coming from dentures, tooth whitening and dental implants.  While revenue distribution is a function of the intended target market of the clinic, it is often correlated with the demographics of patient populations surrounding the dental practice – more on this later.

    Getting you and your dentists onto insurance panels is an important consideration at the setting up phase as well.  Some dental procedures are inherently complex and patients would have them covered under personal or corporate insurance schemes.  By becoming a panelled member will help ease payment arising from these dental procedures and ensure income in the long run.

  • Know your Expenses – the other side of the financials involves where you spend the money. In most cases, the largest component of the clinic’s expenses is the manpower costs – salary, bonus, subsidies, etc. for the staff – dentists, nurses, clinic manager and specialist technicians.  Rental can be a large part of the expenses too.  Depending on the location of your clinic, the rental may vary greatly – from a few thousand dollars per month in public residential estates to tens of thousands of dollars in premium shopping or commercial malls.

  • Know your Service Experience – Patient Service User Experience, commonly called UX, is about knowing and empathizing with the users of the clinic service. As a service provider, the dental clinic’s most important value proposition is not dependent on the latest treatment technologies but in the delivering of the experience that the patients need and want most.  Mapping out your patients’ clinic journey is the first step in your proactive interventions to create a positive and memorable service experience.  Training yourself and your staff to be sensitive to the needs of your patients – be they children, adults or the elderly – will truly transform and elevate your dental practice. 
3. STAFF & SECURITY – Your People and Patients Matter.
  • Your Staff matters – Recruiting, training, assessing and retaining clinic personnel are absolutely critical in the effective operating of your dental practice. Beyond the dentists’ expertise, your frontline service staff that interact with and follow up on treatment plans for your patients is integral to your clinic’s service experience.  They are the links to gain positive reviews and build your practice’s long-term credibility and reputation.  Train your staff at least one month before the clinic commences – this will give you ample time for operation simulation, run-in and feedback.  Give time for the staff to be familiar with your personality and work style – this will allow them to anticipate your needs when workload increases.

  • The Security and Privacy of your patients matters – Ministry of Health has put in place legal regulatory structure for the better management of your patients’ information, and this is to help all parties within dental health system, public and private, to provide holistic dental care for the patients. The Personal Data Protection Act (PDPA) has been enacted since 2012 to ensure that the patients’ information is protected. This should be considered in relation to the National Electronic Health Record (NEHR) provision that requires mandatory accessibility to patients’ record within the healthcare framework.  More on this in section 7 of this article.  Remember to attend PDPA training to be familiar with the Act’s guidelines.

4. SITE – Selecting the Location of Your Practice

Where should you locate your dental clinic?  Finding and deciding on the place and location of your clinic are decisions that must align with your corporate vision and values. Often, the categories of dental service that you offer are determined by the needs and patterns of the population surrounding your clinic.

  • Population Demographics – the population distribution of Singaporean estates needs to be considered when you are practicing as a dental specialist.  Mature estates, with a large population of seniors will require more of geriatric dental care, oral reconstructive procedures and extensive dental interventions; younger estates with young working families will have need of paediatric dental services and aesthetic related or maintenance dental services such as scaling and polishing.  For example, based on Singapore Statistics’ Data on Population Census 2020, in Toa Payoh, with a total of 121,850 residents, around 22% were above 65 years old.  By inference, 1 in 5 of the estate’s patients will likely to be senior and needing dental treatment regimen tailored for the seniors.  In contrast, Punggol, with a population of 174,450, 7.7% were above 65 years old.  The dental services could thus be customized for the children, adults and seniors.

  • Population density of the estate – this will help you assess your competition – the denser the estate, it is likely that you are able to reach more patients within a circle of 1km diameter within your clinic.  This is because the estate’s amenities are planned to be accessible within 15 minutes of walking distance, ensuring that there is sufficient motivation to get to the dental services.
5. SPACE – Layout, Design & Equipping the Dental Clinic

The design, construction of the clinic space and equipment set up – layout, capital outlay, colours and lighting – is one of the most pressing matters during the initial set up phase. It is also the most resource-intensive phase in your dental clinic operation.

  • Layout and design of the dental clinic – how much space is needed for your dental clinic?  If you operate as the sole dentist or with a partnering dentist, the treatment and X-Ray rooms’ space usually occupies 45% of the entire floor area.  The remaining space – 25% for reception and common space, 15% for circulation ( corridor and walking space ) and 15% for storage, pantry and ancillary space – can be considered.  For example, a 2-dentist set up would require an optimal space of 1,000 square feet to be feasible, though it depends entirely on your practice’s objectives.   In addition, the lighting of the spaces within the dental clinic has to be carefully planned.  For example, for the reception and common spaces, a light intensity of 500 – 800 lux would be sufficient.  Within the treatment room, higher ambient lighting intensity of 1000 to 2000 lux would be needed for the dentists to carry out general tasks.  An even higher lighting level from 10,000 to 50,000 lux will be required near the patient’s area for the dentist to perform treatment procedures.  Generally, an ambient to focused lighting ratio between 1:5 to 1:10 would be sufficient to allow the dentist to operate without visual fatigue. 

    Communicate these requirements to your designer / contractor before the clinic’s fitting out.  What about the cost of constructing the clinic space?  The cost of fitting out a dental clinic, excluding the dental equipment, could vary anywhere between $90 per sq ft to $150 per sq ft – depending on the type of materials, finishings and ambience you like to create for your dental space.  The final cost of fitting out the clinic space would be in the upward of $200k for a 1,000 sf clinic, including fees for inspection, engineer’s submission for approvals and miscellaneous charges.  Established local banks such as DBS and OCBC provide renovation loans and extend credit line to assist entreprenuerial dentists during their start up phase.

  • Dental Equipment – one of the largest investments you will make in your dental clinic will be in the dental equipment – dental chairs, X-Rays machines, treatment tools and implements.  It is important that you obtain these equipment from HSA-approved vendors with proper licensing. For example, reputable dental equipment suppliers such as A-dec(USA), Dentsply Sirona (USA) and Takara Belmont (Japan) are represented in Singapore and have wide ranges of dental chairs costing from a few thousand dollars a unit to an upward of $30k per unit.  Beyond styling and features of the equipment, aftersales support and service is one the key considerations when procuring your equipment – a strong local distributor presence will ensure that your equipment is well maintained and operational at all times. In addition, as some equipment weighs in excess of 200 kg ( some heavy-duty models may weigh more than 300 kg ) and needs to be anchored beyond 150 mm into the floor slab for stability, it would require clearance from the landlord or the building’s structural engineer. 

    Dental X-Ray machines belong to a controlled classification, and its operation must be licensed by the National Environment Agency (NEA) due to its radioactive nature.  The space that the X-Ray machine operates in should also be lined with lead panels to contain the radiation.   Your clinic contractor will need to seek the appropriate approval for the design, layout and installation of the space and equipment.
6. SELL – Marketing Your Dental Services

How do you market your dental practice? Ministry of Health has provided a set of comprehensive marketing and advertising guidelines for medical practitioners covering from the installation boundaries of the promotional materials to the wording and design of the marketing content.

  • Branding and visual identity – your clinic’s name, logo and visuals are your corporate spokespersons. They bear the vision, values and promises of the clinic even before your patients enter your clinic.  The design of these visual assets must reflect what the clinic is and believes in, and ought to be applied consistently across all the marketing platforms – the clinic’s website, social media, signage, marketing collaterals – letterhead, envelope, folders, memo pads, name cards, etc. – and even the clinic’s space.

  • Physical Marketing – the clinic space – exterior and interior – is the most prominent visual asset of the practice. The visual design of the space, as mentioned earlier, is vital to retaining your potential patients.  The advertising messaging around and within the clinic, is the other soft asset that persuades your patients – these could be in-clinic brochures, pamphlets, on-screen messages, posters, clinic’s and dentists’ credentials, etc.  The Healthcare Services Act (HCSA) provides comprehensive guidelines on how physical advertisements may be implemented within and around the clinic’s space.

  • Digital Marketing – the digital space is where your potential patients seek out your dental service online. Gradually coming to the fore since the turn of the century, digital marketing has become one of the largest engines that drive medical practices around the world.  In Singapore, our well-connected communities will search out, review and compare services on the internet search space before they commit and arrive at your clinic.  Digital marketing works on the premise that the virtual space that your clinic operates in can be expanded according to the time and financial resources that you commit into building it.

7. SYSTEM –Connecting Your Clinic Information Together

What are the software and information systems that you need to set up in your dental clinic? To ensure seamless clinical operations and strict regulatory compliance, your digital architecture must be built on three core pillars: a dental-specific Clinic Management System (CMS), an automated accounting platform, and direct National Electronic Health Record (NEHR) integration.

  • Dental-Specific CMS – Invest in a HIB-compliant CMS. Dental clinics require specialised workflows, including interactive odontograms (dental charting), periodontal charting, and direct integration with digital imaging software (PACS) for X-rays and OPGs. Crucially, your CMS must be SmartCMS-certified by Synapxe. This ensures native integration for real-time CHAS and MediSave subsidy checking and direct claims submission. Several Clinic Management Systems (CMS) and Practice Management Systems (PMS) in Singapore are equipped with robust, interactive dental and periodontal charting, along with direct integrations for digital imaging and PACS. Leading platforms adopted by local clinics include SGiMEDUNO Technologies, and Plato Medical.

  • Mandatory NEHR Contribution – Under the Health Information Bill (HIB) passed in January 2026, contribution to the NEHR is a mandatory legal requirement for all licensed dental clinics, with enforcement slated for early 2027. Your CMS must automatically push key clinical data—including drug allergies, chronic diagnoses, and active medications—to the NEHR.  Direct integration allows dentists to review a patient’s central medical history, flagging critical risks like blood thinners (crucial before extractions) or bone-modification therapies.  Eligible dental clinics can tap into the NEHR Connect Grant starting in 2026 to offset these onboarding costs.

  • Accounting System – To maintain financial health, bridge your CMS to a cloud accounting platform like Xero via API. This linkage should automatically synchronise daily cash collections, corporate dental plan receivables, and CHAS/MediSave disbursements directly into your ledger, simplifying IRAS tax compliance and vendor payments.
